How to Order:

Fill out our interactive order form with the sale items you’d like to purchase (you may need to update to the latest version of Adobe Readerin order to save your entries). Save it to your computer and email the completed form to manualorders@thrivelife.com. You may also print and scan your completed order form or fax it to 801-492-3978.
Your order form must be submitted by 11:59pm MDT on 10/31 in order to receive sale prices. Sale prices cannot be redeemed online.
You will receive confirmation that your order form has been received within 48 hours, and will receive your order confirmation with your products and order total once your order has been processed, which may take up to 7 to 10 business days.
